By filling in my information and submitted this form:
 

• I agree that ID sessions will be conducted digitally, (via Zoom Pro), and that, by logging in at the agreed time, I confirm that I will follow these guidelines for the session:
   

• I agree that I will not have anyone else in the room with me, nor will I share my computer with anyone else during any ID sessions that I attend.
   

• I understand that attending an ID session, as practiced by the Institute, and affiliated facilitators, inspired by the IoPT method development by Professor Franz Ruppert, is not a medical procedure, nor is it psychotherapy nor a substitute for psychotherapy.
   

• I confirm that I have adequate support in my life to enable me to process anything that may come up for me during an ID session, for example personal therapy, medical supervision, supportive friends, and so on.
   

• I agree to confidentiality about any personal information that emerges during an ID session/resonance process. This includes any form of picture/recording/digital storing or publishing of the process, unless agreed upon in a separate agreement between facilitator, subject and resonators.

• 


• I understand that attending an ID session may involve emotional experiences and I accept full responsibility for my participation and any consequences it may have for me.
   

• I agree that I take full responsibility for myself before, during and after each ID session, and that any discomfort may be addressed by the facilitator, or if needed by my therapist, doctor or other relevant health professional. 

   

• Should the internet connection be broken, (and the internet stays down so that the process must stop,) the facilitator will immediately contact the subject by phone and the resonators will get an sms. The resonators will be taken out of the resonance and be informed that they may call the facilitator when the process has ended, if they have problems getting out of resonance. The facilitator will not be blamed for the effects of problems with internet access and / or software, as this is part of the risk included in using digital medium. 

   

• I am aware that digital and other documentation about internet processes may be deleted after 5 years. 

   

• By logging into an ID session I confirm that I have received enough general information about the ID method, the model of splitting and of the specific adjustments of ID sessions. 

   

• By logging into an ID session I understand that any kind of  aggression or physical acting out during the process is prohibited. The resonance / impulses in this regard can be described verbally.
   

• As a subject or resonator, I understand that I am not to leave an ID session. I am not to disrupt the process or stop it, without this possibility having been first agreed upon with the facilitator in advance.
   

• It is my responsibility to inform the facilitator about any drugs or medicines used before the process. 

   

• I understand that, should I feel the need, I can follow up my experience with the facilitator by email as well as by a scheduled phone call.
   

• I understand that by completing this form I am making these commitments for any online event with facilitator, Helena Gorka or through her assistants.
   

• I agree to indemnify and hold harmless, Helena's affiliates, and its respective officers, directors, agents and employees from any and all claims, demands, losses, causes of action, damage, lawsuits, judgments, including attorneys’ fees and costs, arising out of my or my family’s use of or presence up on the facilities of.
   

• Any equitable or legal claim or claims that may arise from the participation in the above shall be resolved under the laws of the State of California.
   

• I have read this document, and I understand it. I further understand that by signing this release, I voluntarily surrender certain legal rights.
